Joining your children in the early stages of development is very rewarding. With Keiki O Ka ‘Āina Family Centers, parent participation helps to create a lasting bond between you and your child.
Obtain a registration form by contacting our main offices at 843-2502 or use our
for numbers to our various programs.
Complete the registration form for the adult and child and return to our offices. If more than one child, a form for each child is required.
And you'll need:
Copy of Birth Certificate for the participating child(ren)
Copy of immunization records for the participating child(ren) including TB clearance for child(ren) over 2 years of age.
Copy of TB clearance for the participating adult(s)
Research Consent Form
Signed Media Release Form
$50.00 registration fee (for preschools only; one per family)
Part B - Screenings
Agency pre-testing for 2 1/2 years through 5 years
Part C - Orientation
Signing a Family Participation Agreement
Completing the CAP ( Care giver Assessment Packet )
Completing the ASQ's ( ages and Stages Questionnaires )
Class Familiarization
When all of the documents from Part A, B and C have been received in our offices you will receive a notification of the time and location of screening (Part B)
Once you have completed PARTs A, B and C we will send you an "Enrollment Confirmation" letter.
