Joining your children in the early stages of development is very rewarding. With Keiki O Ka 'Āina Family Centers, parent participation helps to create a lasting bond between you and your child.

Here is a brief overview of what it takes to register in our programs.

Obtain a registration form by contacting our main offices at 843-2502 or use our contact page for numbers to our various programs. This registration checklist will be included in your registration packet.

Completed application forms for both child(ren) and participating adult(s).

Copy of Birth Certificate for the participating child(ren)

Copy of immunization records for the participating child(ren).

Copy of participating child(ren) TB clearance (within the 2 last years) unless under 2 years of age

Copy of participating adult(s) TB clearance (within the 2 last years)

Signed Media Release Form

$50.00 registration fee (for preschools only; one per family)

Agency pre-testing for 2 1/2 years through 5 years

Completing the CAP ( Care giver Assessment Packet )

Completing the ASQ's ( ages and Stages Questionnaires )

Signed Research Consent Form

Orientation

When all of the documents from Part A have been received in our office you will receive a notification of the time and location of screening and orientation (Part B)

Once you have completed all of the above we will inform you as to when you may begin attending.
